The association between the Human Development Bonus and chronic child malnutrition in Ecuador was analyzed

Abstract
Chronic child malnutrition remains a persistent problem in Ecuador and disproportionately affects households facing economic, territorial, and housing deprivation. This study aimed to analyze the adjusted association between reported receipt of the Human Development Bonus or pension and chronic malnutrition among children under five. Microdata from the second round of the National Survey on Child Malnutrition were analyzed using a cross-sectional observational design. Height-for-age Z-scores were recalculated according to World Health Organization standards, a housing index was constructed through principal component analysis, and inverse probability weighting with regression adjustment was applied. Children living in recipient households had a 3.97-percentage-point higher adjusted probability of chronic malnutrition and a height-for-age Z-score 0.1365 standard deviations lower than comparable children in non-recipient households. These findings reflect the program’s concentration among households experiencing persistent structural vulnerabilities and do not demonstrate an adverse causal effect of the transfer. Cash transfers should therefore be complemented by maternal and child health, nutrition, water, sanitation, and territorial development interventions.
